Insurance Card or Labor & Industry Paperwork

Please Note: our insurance list is not all inclusive. If your insurance is not listed please feel free to contact our front office and we will assist you in clarifying your eligibility of coverage with Lake Washington Physical Therapy. We will also verify every patient's insurance benefits prior to treatment in our office. Lake Washington Physical Therapy no longer bills secondary insurance plans but will give patients the necessary information so they can submit.

 

Insurance verification is not a guarantee of benefits and each patient must personally verify insurance with their respected insurance to ensure 100% accuracy of benefit coverage. If a patient can not receive reimbursement for rehabilitation at our clinic , we will inform the patient and direct them to the appropriate facility. Thank you for your patience and support.

Co-Pay & Co-Insurance

This is the amount due at the time of service as required by your health insurance contract. In the current healthcare system co-pays are very common and nearly all plans require one. If your plan has a co-pay it is our contractural obligation to your payer to collect this payment. If a plan does not have a co-pay it may institute a co-insurance payment. What this means that for the services rendered the patient may be responsible for a percentage of the insurance companies contracted rate. We recommend comfortable and loose clothing that will provide us the ability to assess area or region of the body where the injury is.
